Epilepsy Queensland Visionary Awards 2022
Award Eligibility
- The Epilepsy Queensland Awards are open to individuals of any age and organisations of any size.
- Please ensure that the individual or organisation being nominated is identified and their activities against the criteria.
- Where the award description mentions ‘immediate epilepsy family’ this refers to an individual who lives with epilepsy and their immediate family / support network.
- Actions/activities of the nominated individual or organisation should be relevant for the current Calendar Year.
- Self-nominations will be accepted.
- References and/or testimonials will be accepted in addition to the award nomination but are not compulsory.
The Hope Award
Awarded to an individual in the immediate epilepsy family who has made a significant contribution to better understanding people living with epilepsy and supporting them to live well.
Nominations should include responses to the following:
- Tell us about the significant contribution that the nominee has made?
- What are the specific outcomes and the impact that has been achieved?
- Describe any obstacles or challenges that the nominee has had to overcome?
- Is there a specific person or group that has benefited from the nominee’s work?
The Hero Award
Awarded to a person or organisation who has made a significant contribution to community awareness, advocating for the removal of barriers or active participation
Nominations should include responses to the following:
- Tell us about the significant contribution that the nominee has made?
- What are the specific outcomes and the impact that has been achieved?
- Describe any obstacles or challenges that the nominee has had to overcome?
- Is there a specific person or group that has benefited from the nominee’s work
The Impact Award
Awarded to the person or organisation who has made a significant contribution to towards making EQ financially strong or has helped us to better understand our impact on the epilepsy community.
Nominations should include a response to one of the following:
- Tell us about the philanthropic contribution of the nominee and why they were motivated to do this work. This could be a single significant donation, a cumulative history of donations or their ability to coordinate and motivate groups of individuals to donate to Epilepsy Queensland.
- Tell us about the voluntary work of the nominee and why they were motivated to do this work.
The Brave Award
Awarded to an individual in the immediate epilepsy family who has shown bravery in championing the cause of those living with epilepsy and working with us to achieve our vision.
Nominations should include responses to the following:
- Tell us about the adversity that the nominee has overcome in their life, or the life of someone close to them, in order to champion the cause of those living with epilepsy?
- What are the specific outcomes and the impact that has been achieved?
- Is there a specific person or group that has benefited from the nominee’s work?
The Agile Award
Awarded to a person or organisation who have demonstrated agility and resourcefulness in adapting to the needs of people living with epilepsy.
Nominations should include a response to one of the following:
- Tell us how the nominated individual has demonstrated agility and resourcefulness in adapting to the needs of people living with epilepsy. This may be an individual within an organisation who has worked to improve the experience of someone living with epilepsy. Please outline any key innovations and creativity that the nominee used that may be relevant.
- Tell us how the nominated organisation has demonstrated agility and resourcefulness in adapting to the needs of people living with epilepsy. This could be by creating employment opportunities for people living with epilepsy or promoting an epilepsy smart workplace.
The Shine Award
Awarded to an individual who has, through their professional or personal endeavours, been a light for people impacted by epilepsy. This might be a clinician and/or researcher who has made a significant contribution to clinical care of epilepsy patients and/or epilepsy research, or an allied health / education professional who has made a significant contribution that benefits and supports the epilepsy community. This award could also be awarded to an individual who has demonstrated a commitment to increasing awareness of epilepsy through writing, speaking, blogging etc.. or an individual from an organisation who has played a major role in developing a valuable partnership with Epilepsy Queensland,
Nominations should include responses to the following:
- Tell us about the significant contribution that the nominee has made?
- What are the specific outcomes and the impact that has been achieved?
- Describe any obstacles or challenges that the nominee has had to overcome?
- Is there a specific person or group that has benefited from the nominee’s work?
